Transcript: Please submit your unofficial transcript from your current college or university. Note: The scholarship recipient will be asked to submit an official transcript verifying enrollment and grade point average.

Personal Statement: On a separate page, write a personal statement about your goals and professional aspirations. In 500 words or less, share the steps you have already taken to accomplish these goals and future steps you will take. This is your chance to share relevant information not contained elsewhere in your application.

Letters of Recommendation: Please arrange to have two (2) letters of recommendation from faculty members or industry professionals sent separately to Scholarship Chairman Brynn DeLange at scholarship@ohiovalleyemmy.org. Please note: At least one letter should attest to character attributes such as integrity and leadership.
